
Affiliation Agreements for UCSF School
of Medicine Training Programs
The Campus Procurement and Business Contracts Office is contracted
by the Office of Medical Education (OME) to review, negotiate, manage,
and archive institutional affiliation agreements with 200+ School of Medicine
training affiliates, ensuring compliance with internal institutional requirements
and external accreditation requirements.
Departments wishing to formalize a training agreement with an affiliate
should complete the School of Medicine Training Affiliation Request Form,
including the required signatures.
Please note:
- New GME rotations must include approval by the GME
office prior to submission of the form to OME.
- Incoming agreements, where trainees from other institutions train
at UCSF, must include approval by the CEO of the Medical Center.
- Request forms should be sent to the Office
of Medical Education, Box 0410. The form must be COMPLETELY FILLED
OUT before OME will sign off.
- Upon approval by the Associate Director of Academic Policy in OME,
the Office of Medical Education will forward the form to the Department
of Campus Procurement and Business Contracts
- The Business Contracts Unit will oversee the rest of the negotiation
process with the affiliate once the request form is approved by OME.
The negotiation process can take from six months to a year to complete.
Departments should bear this in mind and begin the process as far in
advance as possible.
