STOR/CDS Accounts and Passwords

What is STOR/CDS?
STOR/CDS is a clinical information system that provides three major services to the campus community - online access to patient results, patient records for clinic visits, and database search reports.

STOR/CDS is available to UCSF Medical Center and UCSF/Mount Zion Medical Center personnel.

Who has access to STOR/CDS?
Third- and fourth-year students are given access to STOR/CDS To activate your access ID#, you must read and sign the "Computer Security and Use Statement." The completed form must be turned into the Office of Curricular Affairs, S-221. The form is also available in the Office of Curricular Affairs. This formality is required because strict confidentiality regarding patient records must be maintained.

What is my STOR/CDS USER ID#?

  • If you activated your STOR/CDS account prior to June 25, 2001 then your ID # is the last four digits of your social security number preceded by "MS", e.g., MS1234.
  • If you activated your STOR/CDs account after June 25, 2001 then your ID # is the same as your SAA User ID.

What is my STOR/CDS Password and
what do I do if I forgot it?

You create your own password the first time you log into the STOR/CDS system. The password must be at least 6 characters long and have at least one number in it. If you forget your password you must have the Office of Curricular Affairs reset the password for you. No one can access your password so a new one must be created. To reset your password call (415) 476-2346 or send an e-mail to mercurio@medsch.ucsf.edu. It takes 24 hours to reactivate an account if you forget your password!

How do I access patient records at SFGH and the SF-VAMC?
Access to Patient Records at the San Francisco General Hospital and the San Francisco VA Medical Center is handled by the department responsible for you and will be set up at that facility.
